Spawned agents need to run CLI commands without permission prompts. Changed
the agentConfig.permissions schema default from undefined to "skip" so
--dangerously-skip-permissions is included by default. The orchestrator
session is hardcoded to "skip" regardless of config since it must always
run ao CLI commands autonomously.

fix: keep Claude Code interactive after initial prompt (#145)
* fix: keep Claude Code interactive after initial prompt

Claude Code's -p flag runs in one-shot mode (exits after responding),
which prevents follow-up messages via `ao send`. Instead, launch Claude
interactively and deliver the initial prompt post-launch via
runtime.sendMessage().

Adds `promptDelivery` property to the Agent interface so each agent
plugin can declare whether prompts should be inlined in the launch
command or sent after the agent starts.

"Paste code here if prompted >" matched the `>` regex. Now checks
the last non-empty line for Claude's specific ❯ prompt character,
and bails early if the OAuth/login screen is detected.

suraj-markup c307ae30e0 feat(agent-codex): implement mtime-based activity state detection
Use the Codex session file's mtime as a proxy for agent activity.
Codex continuously appends to its rollout JSONL while working, so:
- recently modified file → active
- stale file (past threshold) → idle
- process not running → exited

This replaces the previous null return with real activity detection,
using the same threshold constant (DEFAULT_READY_THRESHOLD_MS = 5min)
as the Claude Code plugin.

Move this.emit("error", err) after the pending-rejection loop in
handleProcessError, matching handleProcessExit's order. Without this,
emit("error") with no listeners throws synchronously, skipping

Three tests relied on the empty default stream coincidentally matching
expected values instead of actually exercising the streaming parser.
Added setupMockStream(content) and stronger assertions to verify the
parser correctly handles missing model, missing token events, and
missing threadId.

1. sessionFileMatchesCwd reads only first 4 KB via open()+read() instead of
   loading entire rollout file into memory (Medium)
2. Binary resolution race condition: added promise guard to prevent
   concurrent resolveCodexBinary() calls (Low)
3. Concurrent connect() calls: added connecting guard to prevent
   orphaned child processes (High)
4. connect() failure cleanup: wrap initialize() in try/catch that
   calls close() on failure (Medium)
5. readline cleanup on process exit/error: close readline in both
   handleProcessExit and handleProcessError (Medium)
6. stderr pipe drain: call stderr.resume() to prevent child blocking
   when stderr buffer fills (High)

All 177 tests pass, typecheck clean.

suraj-markup e410079026 feat(agent-codex): mature Codex plugin to match Claude Code support
Brings the Codex agent plugin to feature parity with Claude Code by
fixing four key gaps and hardening the shell wrapper infrastructure.

## Changes

**`packages/plugins/agent-codex/src/index.ts`**

- Permission flag: `--approval-mode full-auto` → `--dangerously-bypass-approvals-and-sandbox`
- Terminal-based activity detection with Codex-specific patterns:
  idle (bare prompts), waiting_input (approval prompts), active (spinners, esc to interrupt)
- PATH-based shell wrappers (`~/.ao/bin/gh`, `git`, `ao-metadata-helper.sh`):
  - `gh` wrapper captures `pr/create` and `pr/merge` output to update PR metadata;
    all other commands use `exec` for transparent passthrough (no stderr merging)
  - `git` wrapper captures `checkout -b` / `switch -c` to update branch metadata
  - Metadata helper escapes sed metacharacters (`&`, `|`, `\`) in values
  - Uses `grep -Fxv` (fixed-string) instead of regex for PATH cleaning
  - Validates `AO_DATA_DIR`/`AO_SESSION` paths to prevent arbitrary file overwrites
    (rejects traversal, resolves symlinks, allowlists `~/.ao/` and `/tmp/`)
- `getEnvironment()` prepends `~/.ao/bin` to PATH for wrapper interception
- `setupWorkspaceHooks()` / `postLaunchSetup()` install wrappers atomically
  (temp file + rename) and append AGENTS.md section
- `getActivityState()` returns `{ state: "exited" }` when dead, `null` when running

**`packages/core/src/lifecycle-manager.ts`**

- Branch-based PR fallback: when `metadata.pr` is missing, calls
  `scm.detectPR()` via `gh pr list --head <branch>` to auto-discover PRs
  for agents without hook systems (Codex, Aider, OpenCode)

## Test coverage

- 85 tests in `agent-codex` (up from 27), replicating Claude Code test patterns:
  detectActivity edge cases, isProcessRunning boundaries, setupWorkspaceHooks
  file I/O behavior, shell wrapper content verification, atomic write validation
- 245 tests in core pass
- All 22 packages typecheck clean

The next page refresh will retry live API calls, so data recovers
as soon as the rate limit window resets.

- Session ID, title, branch, PR number/link
- Session activity status (working/spawning/etc.)
- Merge button if mergeability was already cached

- cache.ts: TTLCache.set() accepts optional ttl override for per-entry control

3. Check common branch names: main, master, next, develop

Now EnvironmentInfo tracks both:
- currentBranch: The checked-out branch (for display only)

GitHub returns `mergeable: null` for merged/closed PRs. The SCM plugin
was misinterpreting this as unknown/problematic status, leading to
false "Merge conflicts" warnings in the dashboard.

Fixed in two layers:
1. SCM plugin: Check PR state first in getMergeability(). Return clean
   result immediately for merged/closed PRs without querying mergeable.
2. Dashboard: Skip merge conflict display for non-open PRs in
   SessionDetail component.

SessionCard already had protection via early return in getAlerts().

Closes: bug described in task description
Tests: Added tests for merged/closed PR cases, all 54 tests passing

Previously, all sessions showed as "idle" on the dashboard because
activity detection was never called. The Agent plugin interface has
detectActivity(terminalOutput) and Runtime has getOutput(), but the
session manager's list() and get() methods never invoked them.

Changes:
- Updated list() to call runtime.getOutput() and agent.detectActivity()
  after checking if runtime is alive
- Updated get() with the same activity detection logic
- Captures last 30 lines of terminal output for classification
- Falls back to "idle" if output capture fails (graceful degradation)
- Agent plugins classify output into: active, idle, waiting_input,
  blocked, or exited

Testing:
- Added test for activity detection in list() with mocked output
- Added test for activity detection in get() with mocked output
- Added test for graceful fallback when getOutput() fails
- Fixed detectActivity mock (sync not async)
- All 24 tests pass

The dashboard now accurately shows whether agents are actively working,
idle at prompt, waiting for user input, blocked, or exited.

**Key Changes:**
- Add "done" and "terminated" to SessionStatus type
- Update getAttentionLevel to correctly classify terminal sessions
- Skip PR enrichment for terminal sessions (merged, done, terminated)
- Implement 60-second TTL cache for PR enrichment data

## What Changed

- **agent-claude-code plugin**: Added `postLaunchSetup` method that writes a Claude Code hook to `.claude/settings.json` and `.claude/metadata-updater.sh` in each workspace
- **session-manager**: Added `AO_DATA_DIR` environment variable to agent sessions
- **Hook script**: Monitors Bash commands and automatically updates metadata on:
  - `gh pr create` → extracts PR URL, sets `pr=<url>` and `status=pr_open`
  - `git checkout -b` / `git switch -c` → extracts branch name, sets `branch=<name>`
  - `git checkout` / `git switch` (existing branches) → updates branch name for feature branches
  - `gh pr merge` → sets `status=merged`

## How It Works

1. When a session is spawned, `postLaunchSetup` runs after the agent launches
2. Creates `.claude/settings.json` with a PostToolUse hook for Bash commands
3. Writes the metadata updater script to `.claude/metadata-updater.sh`
4. The hook fires after every Bash command execution
5. Parses command and output to detect git/gh operations
6. Updates the session metadata file directly using atomic file operations

## Benefits

- Dashboard shows correct PR associations immediately without manual intervention
- Branch tracking stays in sync with agent actions
- Orchestrator has accurate session state for automation and notifications

Prateek 95dfaa4a7d fix: don't report CI as failing for merged/closed PRs
getCISummary() was returning "failing" whenever getCIChecks() threw an
error, even for merged PRs where GitHub may not return check data.
This caused the dashboard to show "CI failing" for merged PRs.

Now checks PR state before fail-closing — merged/closed PRs return
"none" instead of "failing".

Prateek 5058c409d5 feat: scaffold TypeScript monorepo with all plugin interfaces
Phase 0 complete. Establishes:
- pnpm workspace with 18 packages (core, cli, web, 15 plugins)
- Complete type definitions in packages/core/src/types.ts defining
  all 8 plugin slot interfaces (Runtime, Agent, Workspace, Tracker,
  SCM, Notifier, Terminal) + core service interfaces
- YAML config loader with Zod validation and sensible defaults
- Plugin registry with built-in discovery
- CLAUDE.md with conventions for spawned agents

All agents can now branch from main and implement their assigned
packages against the interfaces defined in types.ts.
