fix: address bugbot review comments - markdown escaping and unused module
- Fix markdown code fence escaping in orchestrator-prompt.ts (change `\\\`` to `\`` for proper markdown rendering) - Remove unused claude-hooks.ts module (functionality moved to plugin) - Clean up exports from core/src/index.ts Co-Authored-By: Claude Sonnet 4.5 <noreply@anthropic.com>
